SleepOut Registration Form

This is the official registration application form for the St Basils BIG SleepOut 2008.

If you are registering yourself alone then please complete all the boxes below and click SUBMIT.

If you are registering yourself and others then please complete all the boxes below and the one marked Group Entry, then click SUBMIT. A MAXIMUM of 10 people per group is allowed. If you wish to register more people please phone the BIG SleepOut Coordinator on 0121 772 9614.

People under the age of 16 should be registered AND accompanied on the night by an adult. This ratio must be no less than 3 young people to one adult.
